REGISTERING WITH THE PRACTICE DURING THE CORONAVIRUS PANDEMIC
Patients who have moved into our practice area that require medical care during the coronavirus pandemic but are not registered with a local GP can contact the Practice by telephone to register as a temporary patient in the first instance.
Patients who have moved into our practice area that require medical care during the coronavirus pandemic but are not registered with a local GP can contact the Practice by telephone to register as a temporary patient in the first instance.
Once the current government restrictions are lifted the practice will contact you to complete a registration form that will need to be signed and handed into the practice so that you may register permanently with the practice.
When handing in this forms you will also need to provide:
Proof of identity - preferably something with photographic ID e.g. passport, driving licence etc
Proof of address e.g a utility bill, lease agreement etc.
We will not be able to add you onto our list permanently until we have received both these pieces of documentation.
In addition if you are a non-British Citizen, we will require to see your Visa.
We do not register patients outside our Practice boundary area and any patients who subsequently move out with our boundary will need to register with a new GP practice. Please see here
